Uninstall Pangram MCP entries

Remove only entries that Pangram CLI still owns exactly.

pangram mcp uninstall --target cursor --dry-run
pangram mcp uninstall --target cursor

An entry with the same name but different command or arguments is left alone and reported as an ownership conflict.
